Accepted Insurance


Our physicians are providers for most major health insurance plans offered in this area. View the list of plans we accept as of October 28, 2012. 

Prior to your appointment in our office, it’s a good idea to check if care by our physicians is covered by your health plan and to learn about your plan’s covered services, payment terms and referral policy.

Please bring your current insurance card with you to your appointments.

Co-payments are expected to be paid at the time of your appointment. For your convenience, our office accepts MasterCard, Visa, Discover, personal checks and cash as payment for services.

Regardless of your insurance coverage, you are responsible for payment for the services performed. Our staff will be happy to submit initial claim forms to your insurance carrier. If you are unable to show proof of coverage, we expect payment for services at the time of your office visit.

As part of OhioHealth, our practice is pleased to offer charity care and financial assistance to patients with limited resources and inadequate medical insurance coverage. The OhioHealth Financial Assistance/Charity program is a sliding scale based upon income and family size from 1 to 400% of the Federal Poverty Guidelines (a sliding scale discount). We also provide care for patients without health insurance at a 35% discount. Payment is due at the time of service.
