Welcome to OhioHealth Marion Area Physicians

Here’s some important patient information to help you prepare for your appointment.

We ask our patients to arrive 15 minutes before their scheduled appointments to allow time for any updates to patient files and other necessary paperwork.

Please bring the following to your appointment:
  • Photo ID
  • Current insurance card(s)
  • Co-payment(s), if required
  • Tests results/films that have been requested by us (if performed outside of OhioHealth)
  • Past medical records you feel are applicable to your health
  • If you have Medicare, your pharmacy insurance drug card

Print a copy and sign our Consent to Treatment and Acknowledgment of Receipt of Notice of Privacy Practices to bring with you on your first visit. 

To update your personal and health insurance information we have on file, go to our Patient Portal.

Appointment Cancellation Policy

If you must cancel your appointment, we’ll be happy to reschedule at a more convenient time. We ask that you give us 24 hours advance notice, providing an opportunity for another patient to been seen.

Prescription Refill Policy

It is our policy to fill routine prescriptions during scheduled office visits. Please check with your pharmacy to see if you have additional refills remaining on your prescriptions. When you call the office for a refill, be sure to have your pharmacy phone number available to give to our staff. You may be due for a follow-up appointment if refills are needed.

Tobacco Free Policy

OhioHealth Marion Area Physicians properties and parking lots are tobacco free and smoking is prohibited.
